The <a href=\"https:\/\/www.fda.gov\/medical-devices\/breast-implants\/risks-and-complications-breast-implants\" target=\"_blank\" rel=\"noopener noreferrer\">FDA<\/a> notes that complication risks increase over time and that a patient may need one or more additional operations during her life.<\/p>\n<p>This does not mean all patients should undergo preventive surgery when an implant reaches a certain age. If imaging shows intact implants, the breasts remain soft and stable, and the patient is satisfied, <strong>replacing normal implants would expose her to new risks of infection, bleeding, capsular contracture, changes in sensation, or malposition without a clear benefit<\/strong>.<\/p>\n<aside class=\"aso-duration__key\">\n      <strong>The key point<\/strong><\/p>\n<p>\u201cNo expiration date\u201d means there is no specific day after which an implant becomes invalid. Despite differences between manufacturers, implant generations, techniques, and published studies, we use <strong>15\u201325 years as an approximate average range<\/strong> in consultations. It is not a median established by a single study, a warranty, or the point at which every implant should rupture.<\/p>\n<div class=\"aso-duration__distribution\" aria-label=\"Examples of possible breast implant lifespans\">\n<article><span>Earlier<\/span><strong>It can happen<\/strong><\/p>\n<p>A complication, rupture, or wish to make a change can lead to surgery after only a few years. Some patients are unlucky and need replacement around year six.<\/p>\n<\/article>\n<article class=\"aso-duration__distribution-main\"><span>Practical guide<\/span><strong>15\u201325 years<\/strong><\/p>\n<p>This is the range we use to set a practical expectation, knowing it brings together very different experiences.<\/p>\n<\/article>\n<article><span>Later<\/span><strong>Also possible<\/strong><\/p>\n<p>Some implants remain intact and satisfactory for decades, and some patients may keep them for the rest of their lives.<\/p>\n<\/article><\/div>\n<p>An average describes a group; it does not predict what will happen to one person. Two patients operated on the same day, with the same model and surgeon, can have different outcomes. The important question is therefore not only \u201cHow old are the implants?\u201d but \u201cWhat is the condition of the implants, capsules, and breasts now?\u201d<\/p>\n<\/section>\n<section class=\"aso-duration__section\" id=\"diez-anos\">\n<div class=\"aso-duration__label\">No automatic replacement schedule<\/div>\n<h2>Do breast implants need to be replaced every ten years?<\/h2>\n<p><strong>No.<\/strong> The historical recommendation to replace all implants at ten years is not applied automatically. Ten years is an important point to check them more thoroughly\u2014in our protocol we perform a follow-up MRI\u2014not a surgical indication on its own.<\/p>\n<div class=\"aso-duration__decision-grid\">\n<article class=\"aso-duration__decision-card aso-duration__decision-card--stay\">\n        <span>Keep and monitor<\/span><\/p>\n<h3>Intact implant, stable breast, satisfied patient<\/h3>\n<p>We continue follow-up. Operating solely because ten years have passed adds risk without a corresponding clinical benefit.<\/p>\n<\/article>\n<article class=\"aso-duration__decision-card aso-duration__decision-card--review\">\n        <span>Consider replacement<\/span><\/p>\n<h3>Rupture, contracture, malposition, or a wish to change<\/h3>\n<p>We tailor the recommendation to the issue, time since implantation, capsule condition, and the patient&#8217;s goals.<\/p>\n<\/article><\/div>\n<p>Replacement surgery does not simply \u201creset the clock\u201d without consequences. New implants can also develop complications, and the operation may require changes to the capsule or pocket. That is why we distinguish between a necessary revision and a preventive operation with no meaningful advantage.<\/p>\n<\/section>\n<section class=\"aso-duration__section\" id=\"seguimiento\">\n<div class=\"aso-duration__label\">Our follow-up protocol<\/div>\n<h2>Annual ultrasound and MRI at ten years<\/h2>\n<p>A silicone implant rupture can be <strong>silent<\/strong>: it does not always change breast shape, cause pain, or become apparent on examination alone. Spain&#8217;s <a href=\"https:\/\/www.aemps.gob.es\/productos-sanitarios\/protesis-mamarias\/informacion-para-pacientes-sobre-protesis-mamarias\/\" target=\"_blank\" rel=\"noopener noreferrer\">AEMPS<\/a> recommends professional follow-up for as long as a patient has implants and explains that ultrasound or MRI can identify problems that are not clinically obvious.<\/p>\n<div class=\"aso-duration__followup\">\n<ol class=\"aso-duration__timeline\">\n<li><span>First year<\/span>\n<div><strong>Postoperative checkups and ultrasound at the clinic.<\/strong><\/p>\n<p>We assess healing, implant position, the capsule, and any fluid collection or finding that needs monitoring.<\/p>\n<\/div>\n<\/li>\n<li><span>Every year<\/span>\n<div><strong>Routine examination and ultrasound.<\/strong><\/p>\n<p>We review our patients annually even when they feel well. An earlier MRI or another test may be needed if there are symptoms, relevant history, ultrasound findings, or diagnostic uncertainty. The <a href=\"https:\/\/acsearch.acr.org\/docs\/3100728\/Narrative\/\" target=\"_blank\" rel=\"noopener noreferrer\">American College of Radiology criteria<\/a> specifically distinguish between patients without symptoms and those with a suspected complication.<\/p>\n<\/aside>\n<\/section>\n<section class=\"aso-duration__section\" id=\"factores\">\n<h2>What can affect how long an implant lasts?<\/h2>\n<p>There is no single reason for replacement. Some factors relate to the device itself; others involve the body&#8217;s response, anatomy, or changes in the breasts over time.<\/p>\n<div class=\"aso-duration__factors\">\n<article><span>01<\/span><\/p>\n<h3>Implant model and generation<\/h3>\n<p>Shell, gel, surface, and manufacturing controls are not identical across models. Data from an older implant should not automatically be applied to a current one.<\/p>\n<\/article>\n<article><span>02<\/span><\/p>\n<h3>Capsule response<\/h3>\n<p>Every implant develops a capsule. If it thickens and tightens, it may harden or distort the breast and sometimes require treatment. This is not \u201crejection\u201d; it is an abnormal scar response.<\/p>\n<\/article>\n<article><span>03<\/span><\/p>\n<h3>Anatomy, placement, and technique<\/h3>\n<p>Tissue coverage, size, pocket, implant position, and appropriately planned dissection affect stability and the visibility of folds or edges.<\/p>\n<\/article>\n<article><span>04<\/span><\/p>\n<h3>Pregnancy, weight, and aging<\/h3>\n<p>Breast tissue and skin continue to change even if the implant remains intact. Sagging, asymmetry, or a mismatch between breast and implant position can develop.<\/p>\n<\/article>\n<article><span>05<\/span><\/p>\n<h3>Trauma and individual events<\/h3>\n<p>A significant impact, infection, seroma, or another complication may require assessment or treatment sooner than expected.<\/p>\n<\/article>\n<article><span>06<\/span><\/p>\n<h3>Patient preferences<\/h3>\n<p>Over time, a patient may change her mind about volume, a natural appearance, or even whether she wants implants at all, despite them remaining intact.<\/p>\n<\/article><\/div>\n<p>The brands we currently use most often\u2014Motiva for mia\u00ae, POLYTECH, and MENTOR in selected cases\u2014have different ranges, surfaces, and warranty programs. Before surgery, we distinguish what an implant can change from what depends on the skin or breast tissue.<\/p>\n<\/div>\n<\/article>\n<article>\n<div class=\"aso-duration__reason-icon\" aria-hidden=\"true\">06<\/div>\n<div>\n<h3>A wish to remove implants permanently<\/h3>\n<p>After years of bodily changes, some patients prefer explantation. We assess whether to remove the implants alone or combine removal with a breast lift, fat transfer, or another reshaping procedure.<\/p>\n<\/div>\n<\/article><\/div>\n<\/section>\n<section class=\"aso-duration__section\" id=\"rotura\">\n<div class=\"aso-duration__label\">A diagnosis does not necessarily mean an emergency<\/div>\n<h2>What we do if an implant has ruptured<\/h2>\n<p>A silicone rupture can remain contained within the fibrous capsule surrounding the implant. This is called an <strong>intracapsular rupture<\/strong>. If it is found in a patient without symptoms, we recommend replacement, but it can usually be arranged as a planned procedure: <strong>it is not a surgical emergency<\/strong>.<\/p>\n<div class=\"aso-duration__rupture-grid\">\n<article>\n        <span>Silent intracapsular rupture<\/span><\/p>\n<h3>Planned replacement<\/h3>\n<p>We confirm the diagnosis, explain the options, and schedule surgery without unnecessary alarm, but we do not recommend ignoring the rupture indefinitely.<\/p>\n<\/article>\n<article>\n        <span>Extracapsular or symptomatic rupture<\/span><\/p>\n<h3>More prompt assessment<\/h3>\n<p>If silicone has escaped the capsule or there is swelling, pain, enlarged lymph nodes, or distortion, we assess the extent and plan removal and replacement according to the case.<\/p>\n<\/article><\/div>\n<aside class=\"aso-duration__protocol\">\n<h3>Should the unruptured implant be replaced too?<\/h3>\n<p>Under our protocol, if one implant ruptures and <strong>both implants have been in place for more than five years<\/strong>, we strongly recommend replacing both. This avoids leaving an implant of the same age and generation on the other side that might require another operation soon afterward. If the implants are newer, we individualize the decision and review the warranty terms as well.<\/p>\n<\/aside>\n<p>The best imaging test depends on the findings. Ultrasound is often the first study, and MRI can confirm or clarify a suspected rupture. A much smaller implant can reveal excess skin; a larger one must not exceed what the tissues can support.<\/p>\n<\/article>\n<article>\n<h3>Remove without replacement<\/h3>\n<p>Explantation can be performed alone or combined with a breast lift and, in selected patients, fat transfer. Replacement may be considered for rupture, significant capsular contracture, malposition, changes in the breasts, or a wish to change or remove the implants.<\/p>\n<\/details>\n<details>\n<summary>What is the lifespan of a breast implant?<\/summary>\n<p>There is no precise lifespan. As a practical clinical guide, we discuss an approximate average of 15\u201325 years. One patient may need replacement much sooner; another may keep intact implants for the rest of her life.<\/p>\n<\/details>\n<details>\n<summary>Do breast implants expire after ten years?<\/summary>\n<p>No. Under our protocol, we perform a follow-up MRI at ten years, but if the implants are intact and the patient is satisfied, we do not recommend replacing them solely because of their age.<\/p>\n<\/details>\n<details>\n<summary>How can I tell if an implant has ruptured?<\/summary>\n<p>Some ruptures cause distortion, pain, or a change in firmness, but others are silent. Feeling well is therefore not enough on its own: clinical and imaging follow-up matter.<\/p>\n<\/details>\n<details>\n<summary>Is an intracapsular rupture urgent?<\/summary>\n<p>A silent intracapsular rupture should be addressed with replacement, but surgery can usually be scheduled and is not an emergency. Symptoms, extracapsular silicone, or inflammation may change the timing and surgical approach.<\/p>\n<\/details>\n<details>\n<summary>If one implant ruptures, should both be replaced?<\/summary>\n<p>In our practice, if both implants have been in place for more than five years, we strongly recommend replacing both. If they are newer, we individualize the decision based on the model, imaging of the other side, and warranty terms.<\/p>\n<\/details>\n<details>\n<summary>Does a lifetime warranty pay for the entire operation?<\/summary>\n<p>Not necessarily. It may cover a replacement implant and, under some programs, limited financial assistance. Surgeon, anesthesia, hospital, and other treatments may not be included. The specific terms must be checked.<\/p>\n<\/details>\n<details>\n<summary>Can I replace my implants with smaller ones?<\/summary>\n<p>Yes. We need to assess the amount of skin and breast tissue and whether the breasts will retain an appropriate shape with less volume or need a lift.<\/p>\n<\/details>\n<details>\n<summary>Can I remove my implants without getting new ones?<\/summary>\n<p>Yes. Permanent explantation is an option. Depending on skin, volume, and changes to the body, it can be done alone or combined with a breast lift and, in some cases, fat transfer.<\/p>\n<\/details>\n<details>\n<summary>How often should I have an ultrasound?<\/summary>\n<p>Our protocol recommends annual ultrasound, in addition to checkups during the first year, and MRI at ten years. Symptoms or an uncertain finding may call for earlier tests.<\/p>\n<\/details>\n<details>\n<summary>Do all implants and brands last the same length of time?<\/summary>\n<p>We cannot assume they do. Model, generation, surface, individual response, implant placement, surgical technique, and changes in the breasts all matter.
