Screening and Safety"},"content":{"rendered":"<div class=\"wpb-content-wrapper\"><p>[vc_row][vc_column][vc_column_text]<\/p>\n<article class=\"aso-mamografia aso-mamografia--2026\">\n<header class=\"aso-mamografia__intro\">\n    <span class=\"aso-mamografia__eyebrow\">Cancer screening, symptoms and implant integrity are different questions<\/span><\/p>\n<p class=\"aso-mamografia__lead\"><strong>Yes: you can have mammograms with breast implants and should follow the screening program appropriate for your age, country and individual risk.<\/strong> Tell the imaging center when booking so an experienced team can take additional views that move the implant back and show as much breast tissue as possible.<\/p>\n<\/header>\n<section class=\"aso-mamografia__answer\" aria-labelledby=\"respuesta-mamografia-protesis\">\n    <span class=\"aso-mamografia__label\">The short answer<\/span><\/p>\n<h2 id=\"respuesta-mamografia-protesis\">Implants do not prevent screening, but they change how the mammogram is performed<\/h2>\n<p>The implant is visible on the mammogram and can obscure some tissue behind it. In addition to standard images, the team usually takes implant-displacement views, also called Eklund views. The implant is gently moved toward the chest wall while the breast tissue is brought forward for imaging.<sup><a href=\"#mamografia-fuente-1\" aria-label=\"Fuente 1\">1<\/a><\/sup><\/p>\n<p><strong>Ultrasound and MRI do not automatically replace mammography.<\/strong> They answer complementary or different questions. This allows time for the adapted protocol. If you know your implant type and its position relative to the chest muscle, share that information and bring previous images if available. The breast team decides whether diagnostic mammography, ultrasound, MRI or biopsy is needed.<\/p>\n<\/section>\n<section class=\"aso-mamografia__section\" id=\"antes-despues\">\n    <span class=\"aso-mamografia__label\">Our preoperative approach<\/span><\/p>\n<h2>What imaging do we request before breast surgery?<\/h2>\n<p>The aim is to assess the breast appropriately for the patient\u2019s age, tissue, history, symptoms and planned operation. There is no single test for everyone.<\/p>\n<div class=\"aso-mamografia__age-grid\">\n<article>\n        <span>Over 35<\/span><\/p>\n<h3>Mammogram and\/or ultrasound<\/h3>\n<p>Our clinic may request mammography, ultrasound or both according to breast tissue, personal and family history, symptoms and recent imaging.<\/p>\n<\/article>\n<article>\n        <span>Under 35<\/span><\/p>\n<h3>Individual assessment<\/h3>\n<p>We consider history, examination, the planned procedure and whether recent imaging is still suitable.<\/p>\n<\/article><\/div>\n<p>When a mammogram is indicated, we aim to obtain it shortly before surgery. This provides a recent baseline and, if there are no symptoms or other medical indications, may allow routine imaging to be deferred while the breast settles after surgery.<\/p>\n<div class=\"aso-mamografia__timeline\" aria-label=\"Timing of imaging before and after surgery\">\n<div><span>Before surgery<\/span><strong>Appropriate imaging and a recent baseline<\/strong><\/p>\n<p>Mammography and\/or ultrasound according to individual assessment.<\/p>\n<\/div>\n<div><span>After surgery<\/span><strong>Usually 9\u201312 months<\/strong><\/p>\n<p>We generally aim to defer a routine mammogram until then when there is no reason to perform it sooner.<\/p>\n<\/div>\n<div><span>If a symptom appears<\/span><strong>Do not wait<\/strong><\/p>\n<p>Diagnostic needs take priority over any routine postoperative timetable.<\/p>\n<\/div><\/div>\n<p class=\"aso-mamografia__microcopy\">Screening ages vary by country and individual risk. Spain approved a gradual expansion of its population program to ages 45\u201374 in 2026; this is not a rule for patients living in the UK or elsewhere in Europe. Follow the screening invitations and risk-based advice in your home healthcare system.<sup><a href=\"#mamografia-fuente-3\" aria-label=\"Fuente 3\">3<\/a><\/sup><\/p>\n<\/section>\n<section class=\"aso-mamografia__section\" id=\"seguimiento\">\n    <span class=\"aso-mamografia__label\">Two follow-up schedules<\/span><\/p>\n<h2>How often should implants be checked with ultrasound or MRI?<\/h2>\n<p>Checks for possible implant rupture <strong>do not replace breast-cancer screening<\/strong>. These schedules may overlap, but they serve different purposes.<\/p>\n<div class=\"aso-mamografia__guidelines\">\n<article>\n        <span>AEMPS \u00b7 Spain<\/span><\/p>\n<h3>Annual clinical review<\/h3>\n<p>Spanish patient information updated in July 2026 recommends annual review with a plastic surgeon while implants remain in place, ultrasound for implant carriers and MRI when needed.<sup><a href=\"#mamografia-fuente-4\" aria-label=\"Fuente 4\">4<\/a><\/sup><\/p>\n<\/article>\n<article>\n        <span>FDA and ACR \u00b7 United States<\/span><\/p>\n<h3>Ultrasound or MRI from years 5\u20136<\/h3>\n<p>For asymptomatic silicone implants, guidance recommends a first ultrasound or non-contrast MRI at 5\u20136 years, then one of these tests every 2\u20133 years. Symptoms or an uncertain ultrasound may warrant MRI sooner.<sup><a href=\"#mamografia-fuente-5\" aria-label=\"Fuente 5\">5<\/a><\/sup><sup><a href=\"#mamografia-fuente-6\" aria-label=\"Fuente 6\">6<\/a><\/sup><\/p>\n<\/article><\/div>\n<div class=\"aso-mamografia__protocol\">\n<div>\n        <span class=\"aso-mamografia__label\">In our practice<\/span><\/p>\n<h3>Annual ultrasound, with MRI around ten years<\/h3>\n<p>We generally perform or request an annual ultrasound. If the examinations remain reassuring, we may suggest MRI around ten years for a more detailed implant assessment. We bring it forward for symptoms, significant trauma, inconclusive ultrasound or suspected rupture.<\/p>\n<\/p><\/div>\n<p>This approach is compatible with US guidance, which accepts <strong>ultrasound or MRI<\/strong> as alternatives for periodic implant checks rather than requiring both each time. Our annual ultrasound is more frequent; MRI adds detail when clinically indicated.<\/p>\n<\/p><\/div>\n<\/section>\n<section class=\"aso-mamografia__section\" id=\"rotura\">\n    <span class=\"aso-mamografia__label\">A common concern<\/span><\/p>\n<h2>Can a mammogram rupture an implant?<\/h2>\n<p><strong>The risk is very low, but it is not zero.<\/strong> Regulators and imaging services recognize the possibility of damage from compression, although reporting systems cannot provide a precise rate. This is not a reason to abandon appropriate cancer screening. Tell the team about your implants and choose a center experienced in imaging patients with them.<sup><a href=\"#mamografia-fuente-4\" aria-label=\"Fuente 4\">4<\/a><\/sup><sup><a href=\"#mamografia-fuente-11\" aria-label=\"Fuente 11\">11<\/a><\/sup><\/p>\n<div class=\"aso-mamografia__experience\">\n      <span>Dr. Aso\u2019s clinical experience<\/span><\/p>\n<p>I have seen a small number of cases compatible with damage during mammography, generally involving old or already deteriorated implants. That experience cannot establish a risk rate or prove the implant was intact before compression.<\/p>\n<\/p><\/div>\n<p>Significant capsular contracture can make displacement views harder and more uncomfortable. Report marked hardness, a change in shape or suspected rupture before imaging so the radiologist can choose the most appropriate assessment.<\/p>\n<\/section>\n<section class=\"aso-mamografia__section\" id=\"deteccion\">\n    <span class=\"aso-mamografia__label\">What the evidence really shows<\/span><\/p>\n<h2>Do implants delay cancer detection or worsen prognosis?<\/h2>\n<p>Silicone implants <strong>have not been shown to increase the risk of ordinary breast-gland cancer<\/strong>.<sup><a href=\"#mamografia-fuente-4\" aria-label=\"Fuente 4\">4<\/a><\/sup> They can obscure some breast tissue on a mammogram and reduce sensitivity, especially if additional views are not taken.<\/p>\n<div class=\"aso-mamografia__evidence\">\n<article>\n        <span>2004<\/span><\/p>\n<h3>Lower mammographic sensitivity<\/h3>\n<p>A large cohort found lower screening sensitivity among women with augmentation, but did not find worse prognostic characteristics in the cancers detected.<sup><a href=\"#mamografia-fuente-7\" aria-label=\"Fuente 7\">7<\/a><\/sup><\/p>\n<\/article>\n<article>\n        <span>2007<\/span><\/p>\n<h3>Series of 4,082 patients<\/h3>\n<p>Women with implants had more palpable lesions and more false-negative mammograms, without significant differences in stage, tumor size, recurrence or survival in that series.<sup><a href=\"#mamografia-fuente-8\" aria-label=\"Fuente 8\">8<\/a><\/sup><\/p>\n<\/article>\n<article>\n        <span>2013\u20132026<\/span><\/p>\n<h3>Observational evidence has limitations<\/h3>\n<p>A 2013 review raised a possible survival concern but noted inadequate adjustment for confounding factors. A 2026 review did not establish a worse prognosis, while emphasizing the need for optimized imaging.<sup><a href=\"#mamografia-fuente-9\" aria-label=\"Fuente 9\">9<\/a><\/sup><sup><a href=\"#mamografia-fuente-10\" aria-label=\"Fuente 10\">10<\/a><\/sup><\/p>\n<\/article><\/div>\n<div class=\"aso-mamografia__note aso-mamografia__note--green\">\n<h3>The useful conclusion is not that implants make no difference<\/h3>\n<p>The more responsible conclusion is: <strong>implants do not increase the usual breast-cancer risk or remove the need for screening<\/strong>, but imaging must be adapted and every new symptom assessed. Continuing screening, sharing prior studies and using experienced centers help prevent avoidable delays.<\/p>\n<\/p><\/div>\n<p>Some studies suggest a lump may be easier to feel when breast tissue lies over an implant. This is not a protective effect and never replaces imaging or medical assessment.<\/p>\n<\/section>\n<section class=\"aso-mamografia__section\" id=\"sintomas\">\n    <span class=\"aso-mamografia__label\">Do not wait for your next routine check<\/span><\/p>\n<h2>Breast symptoms with implants: what should be assessed?<\/h2>\n<p>Warning signs are largely the same with or without implants. An implant adds other possible explanations\u2014contracture, rupture, fluid or displacement\u2014but a new change should not be dismissed as \u201cjust the implant\u201d without assessment.<\/p>\n<div class=\"aso-mamografia__alerts\" role=\"list\">\n<article role=\"listitem\"><span>Lump<\/span><\/p>\n<p>In the breast, near the implant capsule or in the armpit, even after a recent normal mammogram.<\/p>\n<\/article>\n<article role=\"listitem\"><span>Skin or nipple<\/span><\/p>\n<p>New retraction, bloody discharge, a persistent skin change or a wound that does not heal.<\/p>\n<\/article>\n<article role=\"listitem\"><span>Breast size<\/span><\/p>\n<p>Late swelling on one side, enlargement or fluid around an implant.<\/p>\n<\/article>\n<article role=\"listitem\"><span>Shape or firmness<\/span><\/p>\n<p>Progressive hardening, a new deformity, pain or a clear change since previous checks.<\/p>\n<\/article><\/div>\n<p>These changes do not necessarily mean cancer, but they need examination and targeted diagnostic imaging. Sudden late swelling, fluid or a mass near the capsule also requires assessment for implant-associated disease. <a href=\"https:\/\/www.doctoraso.es\/blog\/seroma-o-liquido-despues-de-un-aumento-de-pecho-con-protesis-que-es-y-como-solucionarlo\/\">Implant-associated lymphoma arises in the capsule and is not ordinary breast-gland cancer<\/a>.<\/p>\n<\/section>\n<section class=\"aso-mamografia__section\" id=\"reconstruccion\">\n    <span class=\"aso-mamografia__label\">A different situation<\/span><\/p>\n<h2>What if implants were placed after a mastectomy?<\/h2>\n<p>Follow-up after breast reconstruction cannot simply copy the schedule used after cosmetic augmentation. It depends on whether one or both breasts were removed, what tissue remains, the type of reconstruction, cancer history and the treating breast team\u2019s plan.<\/p>\n<p>After bilateral mastectomy, routine mammography of reconstructed breasts may not be indicated. If a natural breast remains after unilateral mastectomy, it usually continues the appropriate screening program. <strong>The treating breast team should set the individual imaging plan.<\/strong><\/p>\n<\/section>\n<section class=\"aso-mamografia__authority\" aria-labelledby=\"doctor-aso-mamografia\">\n<div>\n      <span class=\"aso-mamografia__label\">Aesthetic and reconstructive breast surgery<\/span><\/p>\n<h2 id=\"doctor-aso-mamografia\">Dr. Jorge Aso: assessing both breast tissue and implants<\/h2>\n<p>I completed Spain\u2019s MIR specialist training in Plastic, Reconstructive and Aesthetic Surgery and hold a doctorate in Medicine, awarded <em>cum laude<\/em>. Follow the screening program appropriate for your age and risk. Ultrasound complements mammography, helps investigate lumps, fluid or lymph nodes, and can assess an implant. Mammography or tomosynthesis remains the basis of routine screening for many patients.<\/p>\n<\/div>\n<\/details>\n<details>\n<summary>Does MRI replace mammography?<\/summary>\n<div>\n<p>Not automatically. Non-contrast MRI for implant rupture and contrast-enhanced MRI for selected cancer-related questions have different purposes. Neither automatically replaces routine mammographic screening.<\/p>\n<\/div>\n<\/details>\n<details>\n<summary>Can a mammogram show a ruptured implant?<\/summary>\n<div>\n<p>It may show some ruptures, especially silicone outside the implant, but cannot reliably rule out rupture contained within the capsule. Ultrasound is often a useful first assessment, with non-contrast MRI when further evaluation is needed.<\/p>\n<\/div>\n<\/details>\n<details>\n<summary>Do implants increase breast-cancer risk?<\/summary>\n<div>\n<p>Silicone implants have not been shown to increase the risk of ordinary cancer in breast tissue. Very rare implant-capsule diseases, including BIA-ALCL, are different conditions with their own symptoms and diagnostic pathways.<\/p>\n<\/div>\n<\/details>\n<details>\n<summary>Can cancer be hidden behind an implant?<\/summary>\n<div>\n<p>The implant can obscure some tissue on standard views. Displacement views improve visibility, but no test is infallible. New symptoms still need investigation even after normal screening.<\/p>\n<\/div>\n<\/details>\n<details>\n<summary>What if I feel a lump and have implants?<\/summary>\n<div>\n<p>Do not wait for the next routine check. Arrange a clinical and diagnostic breast assessment. A lump may arise from breast tissue, the implant capsule, rupture or another cause; imaging and sometimes biopsy are needed to identify it.<\/p>\n<\/div>\n<\/details>\n<details>\n<summary>What imaging is requested before augmentation?<\/summary>\n<div>\n<p>For patients over 35, our clinic may request mammography and\/or ultrasound according to breast tissue and history. For younger patients, we individualize imaging based on history, the planned operation and recent studies.<\/p>\n<\/div>\n<\/details>\n<details>\n<summary>When can I have a mammogram after surgery?<\/summary>\n<div>\n<p>If there are no symptoms or other medical reasons to bring it forward, we aim for appropriate imaging shortly before surgery and usually defer the next routine mammogram for around 9\u201312 months. Diagnostic concerns must be assessed sooner.<\/p>\n<\/div>\n<\/details>\n<details>\n<summary>How often do you check implant integrity?<\/summary>\n<div>\n<p>Our practice generally performs or requests annual ultrasound and may suggest MRI around ten years if the checks remain reassuring. Symptoms, trauma, suspected rupture or an uncertain ultrasound may bring MRI forward.<\/p>\n<\/div>\n<\/details>\n<details>\n<summary>Is follow-up the same after cancer reconstruction?<\/summary>\n<div>\n<p>No. It depends on the mastectomy, remaining tissue and treatment received.
