Technique, placement, implant volume, skin elasticity, existing breast tissue, and the inflammatory response mean each patient progresses at a different pace.<\/p>\n<div class=\"aso-volume__timeline\">\n<article><span>First 24\u201372 hours<\/span><\/p>\n<h3>More swelling and tightness<\/h3>\n<p>The breasts may look high, firm, round, and larger than expected. With mia\u00ae and Preserv\u00e9&#x2122;, fluid infiltrated during the procedure also contributes.<\/p>\n<\/article>\n<article><span>First week<\/span><\/p>\n<h3>Very rapid changes<\/h3>\n<p>Tightness and some edema decrease. One breast may settle before the other, and a temporary difference does not, by itself, predict permanent asymmetry.<\/p>\n<\/aside>\n<\/section>\n<section class=\"aso-volume__section\" id=\"mia-preserve\">\n<div class=\"aso-volume__mia\">\n<div>\n<div class=\"aso-volume__label\">Important for minimally invasive techniques<\/div>\n<h2>With mia and Preserv\u00e9, the first hours can be particularly misleading<\/h2>\n<p>These techniques involve infiltrating tissues with anesthetic solution and saline to allow less traumatic preparation. In our practice, we commonly use <strong>around 200 cc of fluid on average<\/strong>, although the amount is adapted to each case.<\/p>\n<p>During the first hours, a 200 cc implant may therefore visually resemble an increase closer to 400 cc because of the infiltrated volume and surgical swelling. This fluid is in the tissues: <strong>it is not part of the implant and is gradually reabsorbed<\/strong>.<\/p>\n<\/p><\/div>\n<aside>\n        <strong>Our clinic&#8217;s protocol<\/strong><\/p>\n<p>We recommend not removing your bra or assessing size during the first week. Appearance can change radically as the hours pass and the infiltration is reabsorbed.<\/p>\n<\/aside><\/div>\n<\/section>\n<section class=\"aso-volume__section\" id=\"forma-inicial\">\n<h2>Why breasts may look high, round, or artificial at first<\/h2>\n<p>After <a href=\"https:\/\/www.doctoraso.es\/en\/breast-augmentation-in-madrid\/\">breast augmentation<\/a> , the gland, fat, and skin swell, as does the muscle when surgery is performed beneath the pectoral muscle. Together, these changes may temporarily increase cleavage fullness, make the implant look higher, and obscure the lower pole&#8217;s natural curve.<\/p>\n<p>In young patients with little breast tissue, tight skin, and firm tissues, adaptation may be slower and the initial shape tighter. With looser skin\u2014after pregnancy, breastfeeding, or weight loss\u2014the implant may integrate sooner. As upper swelling decreases and the lower pole adapts, the implant looks less high and better centered relative to the areola.<\/p>\n<div class=\"aso-volume__compare\">\n<article class=\"aso-volume__compare-normal\">\n<h3>Normal settling<\/h3>\n<ul>\n<li>Changes progress gradually over weeks or months.<\/li>\n<li>The upper pole softens and the lower pole develops a more natural curve.<\/li>\n<li>The breast maintains a balanced relationship between implant, fold, and nipple.<\/li>\n<\/ul>\n<\/article>\n<article class=\"aso-volume__compare-alert\">\n<h3>Malposition or bottoming out<\/h3>\n<ul>\n<li>The fold moves downward excessively or progressively.<\/li>\n<li>The nipple-to-fold distance increases too much.<\/li>\n<li>The nipple looks too high or the breast loses support.<\/li>\n<\/ul>\n<\/article><\/div>\n<h3 class=\"aso-volume__subheading\">Dual plane, the lower pole, and visual nipple elevation<\/h3>\n<p>In dual plane surgery, the lower pectoral attachments are released so the implant can relate more naturally to the gland and lower pole. With anatomical implants, greater lower projection and this release may encourage progressive expansion of the nipple-to-fold distance.<\/p>\n<p>We prefer to describe this as <strong>relative elevation, or an optical effect<\/strong>: as the lower pole fills and lengthens, the areola looks more centered and forward-facing. In certain anatomies and with some techniques, an anatomical implant can direct more volume toward the lower pole, but the result depends on the whole implant, placement, dissection, and tissues.<\/p>\n<\/aside>\n<\/section>\n<section class=\"aso-volume__section\" id=\"volumen-real\">\n<div class=\"aso-volume__label\">Three different concepts<\/div>\n<h2>The implant does not lose volume: tissues, shape, and perception change<\/h2>\n<div class=\"aso-volume__concepts\">\n<article><span>01<\/span><\/p>\n<h3>Implant volume<\/h3>\n<p>An intact implant retains the volume it was manufactured to contain. It does not \u201cdeflate\u201d during normal recovery.<\/p>\n<\/article>\n<article><span>02<\/span><\/p>\n<h3>Tissue volume<\/h3>\n<p>Edema, infiltration, and swelling do decrease. This is why the contour during the first days does not represent the result.<\/p>\n<\/article>\n<article><span>03<\/span><\/p>\n<h3>Apparent volume<\/h3>\n<p>The same volume can look smaller when it is distributed more naturally and the patient has become accustomed to her new appearance.<\/p>\n<\/article><\/div>\n<p>In consultation, we compare it to a new television: during the first few days it looks enormous in the living room, but over time the eye adjusts even though its dimensions have not changed. It helps explain tendencies, but cannot provide one identical figure for every patient or directly compare a textured anatomical implant with a smooth round one, complete submuscular placement with dual plane, or conventional surgery with mia or Preserv\u00e9.<\/p>\n<div class=\"aso-volume__evidence-grid\">\n<article>\n        <span>Ji et al., 2014<\/span><\/p>\n<h3>Anatomical implants in dual plane<\/h3>\n<p>In 13 patients, measured volume did not decrease between the first month and one year. By six months, size is usually fairly stable and, in some patients, final shape takes up to twelve months.<\/p>\n<\/aside>\n<\/section>\n<section class=\"aso-volume__section\" id=\"casos\">\n<h2>Practical cases: the same breasts at one month and one year<\/h2>\n<p>These examples from our clinic show augmentation with anatomical implants and dual plane technique. Swelling and muscle relaxation do not always progress identically. Su experiencia en cirug\u00eda reconstructiva es muy amplia, habiendo trabajado en el Hospital Universitario del Ni\u00f1o Jes\u00fas y en el Hospital Universitario Doce de Octubre como facultativo especialista. Ha trabajado en estos hospitales p\u00fablicos durante m\u00e1s de 15 a\u00f1os. Tambi\u00e9n es todo un referente en la ense\u00f1anza, habiendo sido colaborador docente en la Universidad Complutense de Madrid durante m\u00e1s de 10 a\u00f1os y Director de Departamento en la Academia AMIR. Su carrera est\u00e1 respaldada por publicaciones cient\u00edficas y conferencias a nivel internacional.
