It can improve a contour, restore lost volume, or complement surgery, but <strong>the volume injected is not the volume that will remain<\/strong>. It is used in aesthetic and reconstructive surgery to add volume or improve shape.<\/p>\n<\/p><\/div>\n<div class=\"aso-lipotransferencia__answer-note\">\n<p><strong>It is your own tissue, not an implant.<\/strong><\/p>\n<p>Some of the graft integrates and some is reabsorbed. Fat transfer adds another step: preparing some of that fat and using it as a graft elsewhere. Lipofilling is another common term for filling an area with your own fat.<\/p>\n<div class=\"aso-lipotransferencia__compare\">\n<div>\n<h3>Liposuction<\/h3>\n<p>Removes fat from a specific area. It does not necessarily mean that fat will be used to fill another area.<\/p>\n<\/div>\n<div>\n<h3>Fat transfer or lipofilling<\/h3>\n<p>Combines removal and reinjection. There is a donor site and a recipient site, each requiring different care.<\/p>\n<\/div><\/div>\n<p>This is not a weight-loss treatment. The amount that can be harvested and safely grafted depends on your anatomy, goals, and the tissues&#8217; capacity to receive it.<\/p>\n<\/section>\n<section class=\"aso-lipotransferencia__section\" aria-labelledby=\"como-se-realiza\">\n<h2 id=\"como-se-realiza\">How is fat transfer performed?<\/h2>\n<p>The procedure has three stages. Although the overall approach is similar, processing, injection plane, anesthesia, and operating time are adapted to the treatment area and any associated surgery.<\/p>\n<ol class=\"aso-lipotransferencia__steps\">\n<li>\n<h3>Harvesting the fat<\/h3>\n<p>Fat is removed with liposuction cannulas from areas such as the abdomen, thighs, or hips. The choice is not based only on where there is excess fat: we also plan the donor area&#8217;s final contour.<\/p>\n<\/li>\n<li>\n<h3>Preparing the graft<\/h3>\n<p>The usable fraction is separated from excess fluid and other components. Settling, centrifugation, or other systems may be used depending on the indication. In our buttock surgery, we use settling. The volume harvested is not the same as the volume finally prepared for grafting.<\/p>\n<\/li>\n<li>\n<h3>Distributing it at the recipient site<\/h3>\n<p>Fat is carefully placed in the appropriate tissues for each region. In the breasts, it is distributed in small deposits at different points; in the buttocks, the injection plane must be exclusively subcutaneous. The aim is not to inject all available fat or concentrate it in a single space.<\/p>\n<\/li>\n<\/ol>\n<\/section>\n<section class=\"aso-lipotransferencia__section\" aria-labelledby=\"Lipotransferencia_en_gluteos_y_senos\">\n<h2 id=\"Lipotransferencia_en_gluteos_y_senos\">Fat transfer to the breasts and buttocks: different goals<\/h2>\n<p>Both procedures use your own fat, but they should not be described as the same operation. Expectations about volume, recovery, and safety should not be transferred from one to the other.<\/p>\n<div class=\"aso-lipotransferencia__subsection\" id=\"lipotransferencia-pecho\">\n<h3>In the breasts: mainly shaping, contouring, and filling<\/h3>\n<p>In our practice, breast fat grafting is used mainly to <strong>complement implants, a breast lift, or implant removal<\/strong>. It can fill specific areas, soften transitions, and improve shape. Its contribution is assessed as part of the overall surgical result.<\/p>\n<div class=\"aso-lipotransferencia__clinical\">\n<p class=\"aso-lipotransferencia__eyebrow\">Dr. Aso&#8217;s approach<\/p>\n<p><strong>We find fat a more effective tool for shaping and filling the breasts than for substantially increasing cup size.<\/strong> We perform relatively few augmentations using fat alone. We explain that the volume gain remaining after each graft has settled is usually small, and repeat sessions are generally needed for a noticeable increase.<\/p>\n<\/p><\/div>\n<p>This does not mean fat cannot add volume; expectations need to be realistic from the outset. We do not promise a specific cup size or equate a given volume of fat with the same volume in implants. After implant removal, a graft should not be presented as automatically replacing all the volume the implants provided.<\/p>\n<p>Fat can complement a <a href=\"https:\/\/www.doctoraso.es\/en\/mastopexy-in-madrid\/\">mastopexy or breast lift<\/a>, but filling does not remove excess skin or replace a lift when one is indicated.<\/p>\n<\/p><\/div>\n<div class=\"aso-lipotransferencia__subsection\" id=\"lipotransferencia-gluteos\">\n<h3>In the buttocks: proportion, contour, and projection<\/h3>\n<p>Buttock fat transfer, also known as a BBL, combines reshaping the donor areas with grafting to improve the relationship between the waist, hips, and buttocks. Design matters as much as volume: not every body can accommodate or needs the same projection.<\/p>\n<div class=\"aso-lipotransferencia__safety\">\n<p><strong>In our practice: exclusively subcutaneous injection with real-time ultrasound.<\/strong> Fat is placed above the gluteal fascia, never in or beneath the muscle. Ultrasound helps visualize the cannula and confirm the plane during injection.<\/p>\n<\/p><\/div>\n<p>These are essential safety measures, but they do not eliminate risk or justify pursuing any requested volume. The aim is to restore proportion and a more rested appearance, not indiscriminately fill every wrinkle. Not all expression lines are caused by volume loss.<\/p>\n<p>Although fat has also been used in the lips, <strong>we generally prefer hyaluronic acid for this area<\/strong> because it can be adjusted and dissolved. Nodules or cysts after fat grafting can be difficult to treat. It changes soft tissue, not bone structure: the result must respect your anatomy, and we cannot promise a silhouette identical to a reference photo.<\/p>\n<\/section>\n<section class=\"aso-lipotransferencia__section\" aria-labelledby=\"candidatos-lipotransferencia\">\n<h2 id=\"candidatos-lipotransferencia\">Who may be a candidate?<\/h2>\n<p>Wanting to remove fat from one area and place it in another is not enough. During consultation, we assess your health, medical and surgical history, available fat, tissue quality, and goals.<\/p>\n<ul class=\"aso-lipotransferencia__checklist\">\n<li><strong>Enough donor fat:<\/strong> it must be possible to harvest it without harming that area&#8217;s contour. A slim person may have enough fat for a small correction but not for extensive body augmentation.<\/li>\n<li><strong>Suitable recipient tissues:<\/strong> not all harvested fat can be safely grafted.<\/li>\n<li><strong>A reasonably stable weight:<\/strong> major changes can alter the result.<\/li>\n<li><strong>Realistic expectations:<\/strong> accepting reabsorption, changes over several months, and the possibility of another session.<\/li>\n<\/ul>\n<p>The advantage is using your own tissue while treating both the donor and recipient areas. Limitations include fat availability, graft variability, and complications at both sites. A natural-looking result is a planning goal, not a guarantee simply because the fat is autologous.<\/p>\n<\/section>\n<section class=\"aso-lipotransferencia__section\" aria-labelledby=\"cuanto-dura-lipotransferencia\">\n<h2 id=\"cuanto-dura-lipotransferencia\">How long does fat transfer last, and how much fat is reabsorbed?<\/h2>\n<p><strong>The part of the graft that integrates may remain long-term.<\/strong> However, not all injected fat survives. During the first months, swelling resolves and some of the graft is reabsorbed: the appearance in the first few days does not predict the final volume.<\/p>\n<div class=\"aso-lipotransferencia__sequence\" aria-label=\"How volume changes after grafting\">\n<div><span>Initially<\/span><\/p>\n<p>Injected fat and swelling both contribute to the visible volume.<\/p>\n<\/div>\n<div><span>Over the following months<\/span><\/p>\n<p>Swelling decreases, revealing how much of the graft remains.<\/p>\n<\/div>\n<div><span>Long-term<\/span><\/p>\n<p>Integrated fat remains living tissue: it can change with weight and aging.<\/p>\n<\/div><\/div>\n<p>No retention percentage can be guaranteed for every person and treatment area. The <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/37166041\/\" target=\"_blank\" rel=\"noopener noreferrer\">systematic review of breast augmentation with fat<\/a> reports variation between studies; its figures do not predict an individual&#8217;s outcome and cannot automatically be applied to the face or buttocks.<\/p>\n<p>\u201cLong-lasting\u201d therefore does not mean that shape and size will stay identical throughout life. Nor does it mean that grafted fat has a fixed expiration date.<\/p>\n<\/section>\n<section class=\"aso-lipotransferencia__section\" aria-labelledby=\"sesiones-lipotransferencia\">\n<h2 id=\"sesiones-lipotransferencia\">How many sessions are needed?<\/h2>\n<p>This depends on your goals, the area, available fat, and how the first graft settles. <strong>Repeating a session to add volume or refine shape is not, by itself, a complication<\/strong>, but it should be discussed before treatment, including the additional recovery and cost.<\/p>\n<p>For breast augmentation with fat alone, our experience is that repeat sessions are generally needed for an appreciable size increase. When fat complements another procedure, success is measured not only by breast size but by improvement in the specific area being corrected.<\/p>\n<p>In the face and buttocks, a second session may refine the result. This is not decided on the basis of early swelling: we assess it once the retained volume can be distinguished. Timing is individualized, particularly when several procedures have been combined.<\/p>\n<\/section>\n<section class=\"aso-lipotransferencia__section\" aria-labelledby=\"Postoperatorio_y_recuperacion_tras_una_lipotransferencia\">\n<h2 id=\"Postoperatorio_y_recuperacion_tras_una_lipotransferencia\">Aftercare and recovery: two areas need attention<\/h2>\n<p>Swelling, bruising, and discomfort can occur where fat is harvested and where it is grafted. Our protocol avoids direct pressure on the buttock graft for the first two weeks.<\/td>\n<\/tr>\n<\/tbody>\n<\/table>\n<div class=\"aso-lipotransferencia__compare\">\n<div>\n<h3>Donor area<\/h3>\n<p>A compression garment may be needed, depending on the liposuction performed. Swelling, bruising, and tenderness evolve over the first weeks.<\/p>\n<\/div>\n<div>\n<h3>Recipient area<\/h3>\n<p>Protection must be adapted to the graft. Compression prescribed for the donor area should not automatically be applied to the recipient area.<\/p>\n<\/div><\/div>\n<p>Walking and gradually resuming activities are part of follow-up, but strenuous exercise requires instructions specific to each operation. <strong>Do not massage the graft or use compression garments or cushions on your own initiative.<\/strong> Lymphatic drainage of liposuction areas and graft care are not the same thing.<\/p>\n<p>Results are assessed over the following months, not when you return to work. Follow-up checks both sites and allows instructions to be adapted to your progress.<\/p>\n<\/section>\n<section class=\"aso-lipotransferencia__section\" aria-labelledby=\"Los_riesgos_tras_una_cirugia_de_lipotransferencia\">\n<h2 id=\"Los_riesgos_tras_una_cirugia_de_lipotransferencia\">What are the risks of fat transfer?<\/h2>\n<p>Using your own fat avoids an implant for that filling, but it does not avoid surgical complications. Risks include infection, bleeding, adverse reactions to anesthesia, seroma\u2014fluid accumulation\u2014irregularities, asymmetry, partial reabsorption, oil cysts, and fat necrosis\u2014loss of viability in some grafted tissue.<\/p>\n<div class=\"aso-lipotransferencia__risk-list\">\n<div>\n<h3>Breasts<\/h3>\n<p>Oil cysts, calcifications, or fat necrosis can cause lumps and imaging findings. Tell your radiology team that you have had fat grafting, and continue the recommended screening and follow-up. A new lump needs assessment, not an assumption that it is simply grafted fat. The <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/31772879\/\" target=\"_blank\" rel=\"noopener noreferrer\">international consensus on breast fat grafting<\/a> highlights the importance of follow-up by breast imaging specialists.<\/p>\n<\/div>\n<div>\n<h3>Buttocks<\/h3>\n<p>Pulmonary fat embolism can occur if fat enters the venous circulation: it is uncommon but potentially fatal. The <a href=\"https:\/\/www.plasticsurgery.org\/for-medical-professionals\/publications\/psn-extra\/news\/gluteal-fat-grafting-a-joint-safety-statement\" target=\"_blank\" rel=\"noopener noreferrer\">joint safety recommendations from plastic surgery societies<\/a> support subcutaneous injection and real-time ultrasound. These measures do not mean zero risk.<\/p>\n<\/div>\n<div>\n<h3>Face<\/h3>\n<p>In addition to irregularities or nodules, serious vascular complications involving vision loss or neurological injury have been reported when fat enters an artery. The <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/37563433\/\" target=\"_blank\" rel=\"noopener noreferrer\">review of arterial embolism after facial fat grafting<\/a> documents these cases; published case reports do not establish an individual&#8217;s complication rate.<\/p>\n<\/div><\/div>\n<p>Patient selection, experience, planning, technique, and follow-up help reduce risks but do not eliminate them. Consent should explain the risks of the specific treatment area and any combined procedures.<\/p>\n<aside class=\"aso-lipotransferencia__alert\" aria-label=\"When to seek medical attention\">\n<h3>Warning signs<\/h3>\n<p>Difficulty breathing, chest pain, sudden vision loss, or neurological symptoms require urgent medical attention. Es Doctor en Medicina \u201cCum Laude\u201d por la Universidad Complutense de Madrid, gracias a su Tesis Doctoral de estudio morfom\u00e9trico facial. Es miembro numerario de la SECPRE (Sociedad Espa\u00f1ola de Cirug\u00eda Pl\u00e1stica, Reparadora y Est\u00e9tica) y del Ilustre Colegio de M\u00e9dicos de Madrid. El Dr. Aso imparte docencia a especialistas en Cirug\u00eda pl\u00e1stica de toda Espa\u00f1a y participa frecuentemente en los congresos m\u00e1s importantes de la especialidad como profesor invitado. Ha sido reconocido como el mejor cirujano pl\u00e1stico en Madrid en diversos premios y menciones, entre ellos por el portal Doctoralia, y gan\u00f3 el Premio al mejor Cirujano Pl\u00e1stico de Espa\u00f1a en 2015. Su experiencia en cirug\u00eda reconstructiva es muy amplia, habiendo trabajado en el Hospital Universitario del Ni\u00f1o Jes\u00fas y en el Hospital Universitario Doce de Octubre como facultativo especialista. Ha trabajado en estos hospitales p\u00fablicos durante m\u00e1s de 15 a\u00f1os. Tambi\u00e9n es todo un referente en la ense\u00f1anza, habiendo sido colaborador docente en la Universidad Complutense de Madrid durante m\u00e1s de 10 a\u00f1os y Director de Departamento en la Academia AMIR. Su carrera est\u00e1 respaldada por publicaciones cient\u00edficas y conferencias a nivel internacional.
