Double Chin Surgery in Madrid
A double chin is not always caused by fat. Skin, chin position, muscle, and deeper structures can completely change which technique will offer a natural-looking result.
A direct answer
What does double chin surgery involve?
It is surgery to define the transition between the chin, jawline, and neck. It may involve liposuction with microcannulas, adding BodyTite to support contraction in appropriately selected skin, correcting insufficient chin projection through chin surgery (genioplasty) , or treating the platysma muscle and deeper neck structures.
That is why I prefer not to describe one universal technique. Two patients describing the same concern may need very different operations. Consultation should identify which layer creates the contour and how much change the person wants.
Diagnosis before technology
Fat, skin, chin, or the deep neck: four different causes
This is the most important part of the assessment. We do not choose BodyTite, a cannula, or a lift because of its commercial name: we choose the procedure that addresses the anatomy.
Superficial fat is the main concern
Double chin liposuction
This is appropriate when there is a localized deposit and the skin retains a good ability to adapt. It uses small concealed incisions and microcannulas.
≈20 % of our current practice, alone or with BodyTite
Fat with limited skin laxity
Liposuction + BodyTite
Internal radiofrequency may support contraction in selected patients. Its effect is not the same in every skin type and does not replace a lift when there is clear excess tissue.
We combine them whenever it can add value, except with excellent young skin
A recessed chin or insufficient support
Chin surgery, with or without liposuction
Advancing the patient’s own chin bone can visually tighten the neck and improve the profile. We almost always move the natural bone; if an implant is ever needed, it is custom-made and never silicone.
≈60 % involves chin surgery, alone or in combination
Bands, deep fat, or significant laxity
Deep neck surgery or a lift
This may require direct access to treat the platysma, subplatysmal fat, and, in specific cases, other deep structures. If there is excess skin, a neck or face-and-neck lift is considered.
≈20 % requires deep neck surgery or a lift
These proportions are approximate and describe our current practice. They are not a rule that applies to an individual patient and do not replace examination.
The consultation that helps avoid incomplete treatment
You do not need to choose a technique online
You need to know whether the concern lies above or below the platysma, whether the skin can contract, and whether a small chin makes the neck look fuller. The recommendation may be simple; what matters is that it follows a complete diagnosis.
- Examination of superficial and deep fat.
- Skin quality, thickness, and ability to contract.
- Chin projection, height, and symmetry.
- Platysmal bands, neck angle, and relationship to the rest of the face.
Our approach
The technique varies; the goal is always a natural-looking neck
We do not simply aim to empty the area. The jaw, chin, and neck should continue to form a coherent transition that suits each patient’s age and features.
Superficial liposuction
After local anesthetic infiltration, fat between the skin and platysma is removed through tiny incisions concealed under the chin and, if necessary, near the earlobes. Removal is gradual to define the angle without creating depressions or abrupt transitions.
- Usual duration
- ≈20 minutes
- Recovery at the facility
- ≈1 hour
- Anesthesia
- Local with sedation, without intubation
BodyTite
After liposuction, we apply internal bipolar radiofrequency when the skin may benefit from additional stimulation. This adds around 30 minutes. I always explain that it may support contraction but cannot promise the result of skin removal.
Chin surgery
When the chin is recessed, advancing its own bone can address the structural cause of a short-looking neck. Planning considers the nose, lips, jaw, and neck as a unit. Explore the chin surgery technique.
Platysma, deep fat, and lifting
In open surgery, we can treat deep fat and muscle under direct vision; in specific indications, the digastric muscles or submandibular glands may also be assessed. A lift is added if there is excess skin. These are longer procedures with a different recovery.
Safety comes first
All operations take place at Hospital Universitario HLA Moncloa
Even a twenty-minute liposuction procedure takes place in hospital with an anesthesiologist. HLA Moncloa has 14 operating rooms for major surgery, a post-anesthesia recovery unit, and 13 intensive care beds; its Intensive Care Unit is accredited to train specialists through Spain’s MIR residency system.
Anesthesia is adapted to the procedure. Liposuction alone and its combination with BodyTite are usually performed with local anesthesia and sedation, without intubation. Chin surgery, deep neck surgery, and lifting are managed under general anesthesia. Most procedures are day surgery; admission is mainly reserved for lifting or deep neck surgery.
Realistic recovery
When can I return to everyday life?
These timings apply to liposuction alone or combined with BodyTite. Chin surgery, deep neck surgery, and lifting each have their own protocol.
Liposuction alone
- 24–48 hContinuous use of the chin strap; showering depends on recovery.
- 2–3 daysIndicative return to office work.
- 48–72 hDriving if there is no discomfort or medication that makes it unsafe.
- 7–10 daysGentle exercise; strenuous exercise usually waits 3–4 weeks.
- ≈3 monthsIndicative timing for the contour to settle.
Liposuction + BodyTite
- First 48 hContinuous use of the chin strap; then for one or two weeks while sleeping.
- 3–5 daysIndicative return to office work and driving.
- Day 3–5Massage or lymphatic drainage if recovery warrants it.
- 10–14 daysGentle exercise; strenuous exercise around one month.
- 3–6 monthsSkin contraction and contour continue to mature.
These figures are indicative. Swelling, bruising, fibrosis, and small initial asymmetries are part of a recovery that should be assessed at follow-up appointments.

Experience and a craniofacial perspective
Meet Dr. Aso
Double chin surgery requires looking beyond a fat deposit. My doctoral thesis studied craniofacial geometric morphometrics, and that broader perspective is particularly useful when deciding whether the chin, jaw, or deep neck matters more than the fat itself.
I perform more than 60 double chin procedures a year and have performed more than 500 over the years, from very brief liposuction procedures to chin surgery, deep neck surgery, and lifts. Experience does not mean always proposing the largest operation: it means recognizing the smallest procedure that may create a convincing change and when that procedure would fall short.
Clinical transparency
The concerns we actually see after double chin surgery
In our experience, we have not recorded nerve injuries or extensive necrosis with lasting effects. This does not mean zero risk: any facial surgery can involve hematoma, infection, seroma, nerve injury, changes in sensation, scarring, asymmetry, or revision surgery.
The situation we encounter most often is not a serious medical complication, but a request to refine a small irregularity or difference between sides. Liposuction works on living tissue: swelling, fibrosis, and contraction are not mathematically identical.
Results that illustrate anatomy
Real cases in which the chin also improves the neck
While we prepare a dedicated double chin liposuction gallery, we show examples from our chin surgery gallery in which chin projection changes the neck profile. These are not presented as cases of liposuction alone.

Tell us what concerns you about your profile
You do not need to know whether you want liposuction, BodyTite, chin surgery, or a lift. That is precisely the decision we will make after assessing your anatomy and the change you want.
We listen to your goalWhat concerns you, for how long, and how much change you want.
We examine four layersFat, skin, chin, and the deep neck.
We propose a proportionate procedureIncluding not operating if the balance is unfavorable.

Frequently asked questions
Questions about double chin surgery
How do I know whether I need liposuction or a lift?
Liposuction is better suited to superficial fat when the skin can readapt. If there is clear excess skin, platysmal bands, or a deeper concern, a lift or neck surgery may be more predictable. BodyTite has an intermediate role in selected patients but does not replace skin removal.
Is double chin liposuction performed under general anesthesia?
In our practice, it is usually performed with local anesthesia and sedation, without intubation. Chin surgery, deep neck surgery, and lifting are managed under general anesthesia. All procedures take place at Hospital Universitario HLA Moncloa with an anesthesiologist.
How long does double chin liposuction take?
Liposuction alone takes around 20 minutes, followed by approximately one hour of recovery. BodyTite adds around 30 minutes. When chin surgery, deep neck surgery, or lifting is combined, duration can range from an hour and a half to six hours when the whole face is treated.
How much does double chin liposuction cost in Madrid?
In our clinic, double chin liposuction alone has an indicative price of €3,500. BodyTite, chin surgery, deep neck surgery, and lifting require a different quote depending on technique, anesthesia, time, and admission.
Does BodyTite prevent loose skin?
It may support contraction in skin with sufficient capacity, but the effect varies. Published studies support improvement in selected patients; they do not allow us to promise that internal radiofrequency will correct excess skin requiring removal.
When is chin advancement recommended?
When a recessed chin contributes to a short or poorly defined neck. In those cases, removing fat without correcting support may give an incomplete result. We almost always move the patient’s own bone; implants are exceptional, custom-made, and never silicone.
When can I work, drive, and exercise?
After liposuction alone, office work is often possible in 2–3 days, driving in 48–72 hours, and gentle exercise in 7–10 days. With BodyTite, we generally discuss 3–5 days for office work or driving and 10–14 days for gentle exercise. These ranges are indicative and change when other procedures are combined.
How long is the chin strap worn?
After liposuction alone, it is worn continuously for 24–48 hours and then for around one week while sleeping. With BodyTite, we usually keep it on for 48 hours, then for one or two weeks while sleeping, adapting the protocol to recovery.
Can a double chin return?
Removed fat does not return in the same way, but remaining cells can enlarge with weight gain. Skin aging and muscle changes also continue. Maintaining a stable weight helps, but no procedure stops the passage of time.
What risks or refinements may occur?
There may be hematoma, seroma, infection, irregularity, asymmetry, fibrosis, changes in sensation, nerve injury, scarring, or revision surgery. In our experience, we have not recorded nerve injuries or extensive necrosis with lasting effects; the most common reason for refinement is a small irregularity or difference between sides.
