Is mia Femtech a nonsurgical treatment or an injectable filler?
It is not a filler treatment. mia Femtech is minimally invasive breast surgery using Diamond silicone gel implants. A purpose-designed delivery device inserts the implant through a small incision in the armpit. No liquid or free silicone is injected: a complete breast implant, specifically designed for this technique, is placed inside a surgically created pocket.
Our usual protocol: the procedure takes place in an operating room with local anesthesia and sedation, usually without intubation or general anesthesia. The anesthetic plan is confirmed after an individual medical assessment; general anesthesia may be appropriate in some cases.

At Dr. Aso’s clinic, mia Femtech® is an advanced approach to breast augmentation that avoids an incision on the breast itself. The implant is inserted through an incision of approximately 2 cm in the armpit crease. There is still a scar, but it is positioned away from the breast and is intended to be discreet.
mia® uses dedicated instruments to place the implant through a small underarm incision without cutting the pectoral muscle. It aims to limit tissue trauma and support a faster recovery in suitable patients. Minimally invasive surgery still requires medical assessment, anesthetic monitoring and postoperative follow-up.
It is designed for patients seeking a subtle, natural-looking change. Suitability depends on your anatomy, health and goals, not simply on wanting a shorter recovery.
How tissue preservation works
Which tissues does mia Femtech® preserve, and how is the implant pocket created?
With mia Femtech®, the implant is placed in a prepectoral intralamellar plane: behind the breast gland and in front of the fascia covering the pectoral muscle.
A narrow channel is created from the armpit to this plane. A balloon then expands in a controlled way to separate tissue layers and create the pocket. After the balloon is removed, the Diamond implant is inserted using its dedicated delivery device.
In a straightforward mia® augmentation, no incision is made on the breast, the gland is not cut, and the pectoral muscle is neither cut nor detached. Tissue preservation does not mean there is no trauma or risk: this is still surgery. Occasionally, a bleeding point may need to be coagulated or a resistant tissue band released in a limited way.

Usually a day-case procedure with sedation
In suitable patients, mia® is performed with local anesthesia and sedation monitored by our anesthesiology team, usually without general anesthesia. It is normally a day-case procedure, with discharge after the team is satisfied that you are ready. Yes. mia Femtech® uses Diamond silicone gel implants designed for this technique. Their shape provides projection with a relatively small implant volume. The procedure aims for a modest, natural-looking enhancement, often described as one to two cup sizes in suitable patients. Cup sizes are not standardized, however, and the outcome depends on your anatomy and the implant selected. These are complete implants, not liquid silicone or injectable fillers.
2. Is 25 minutes enough time for the operation?
In selected patients, approximately 25–30 minutes is the usual skin-to-skin operating time. Anesthetic preparation, monitoring and recovery make the overall stay longer. A shorter operation does not, by itself, demonstrate greater safety.
The observations reported in our clinical series are preliminary and reflect our own experience. They should not be treated as proof that mia® is safer than every conventional technique. Appropriate indications, careful patient selection and follow-up remain essential.
3. Will I feel pain if I do not have general anesthesia?
Local anesthesia and sedation are used to keep you comfortable during the procedure, with monitoring by the anesthesia team. You may feel sleepy, but this is not the same as general anesthesia. Sedation and local anesthesia also have risks, which the team will discuss with you.
In particular circumstances, general anesthesia may be more appropriate, depending on your medical history and the anesthesiologist’s assessment. Discuss any concerns about pain, awareness or anesthesia before surgery so that the plan can be tailored to you.
4. How does mia® differ from conventional breast augmentation?
The difference is the combination of access, implant plane and instruments. mia Femtech® uses a small underarm incision, a narrow channel to the prepectoral intralamellar plane, balloon expansion of the pocket and a dedicated implant delivery device. In a straightforward augmentation, this avoids an incision on the breast and cuts to the gland or pectoral muscle.
A conventional breast augmentation does not always involve cutting the muscle either: several implant planes are available, depending on anatomy. mia® focuses on tissue preservation and controlled pocket expansion, which may limit swelling and support recovery in selected patients. It does not guarantee freedom from pain, changes in sensation or complications.
5. What is the purpose of the biconvex Diamond implant design?
Diamond implants have two curved faces, rather than a flat back and curved front. This shape is intended to provide projection with a modest volume. Because both faces are curved, front-to-back orientation differs from that of a conventional implant with a flat base. The design is not, by itself, a guarantee of safety or freedom from implant-related complications.
The aim is a proportionate enhancement without an excessively full appearance. Your anatomy and tissue coverage remain important in selecting an implant.
6. How does mia Femtech® affect the choice of implant size?
mia Femtech® emphasizes breast shape and projection rather than volume alone. Our current Spanish treatment information describes implants of up to 195 cc for this technique. Their appearance cannot be converted reliably into a fixed equivalent size for another implant: shape, dimensions and your starting anatomy all matter.
During consultation, we assess the change you would like and whether a subtle enhancement can meet your expectations. We do not promise a particular cup size.
7. Am I a candidate for mia®?
mia® may suit patients seeking a modest, natural-looking increase rather than a dramatic change. Wanting less invasive surgery or a shorter recovery is not enough to establish suitability: we also assess tissue coverage, breast shape, medical history and your goals.
If you would like a different degree of enhancement, Preservé or conventional augmentation may be alternatives. The appropriate technique and implant range are discussed after assessment.
A consultation helps you compare options and understand their benefits, limitations and risks. It does not commit you to surgery.
8. Could mia® affect breastfeeding in the future?
In a straightforward mia® augmentation, the implant is placed behind the breast gland without cutting through it or using an incision around the areola. This tissue-preserving approach is intended to avoid disrupting the gland.
No breast operation can guarantee future breastfeeding. If difficulties occur, both maternal and infant factors should be assessed, as they would be for someone who has not had surgery. Read our guide to breastfeeding with implants.
An implant can obscure some breast tissue on standard views, but it does not remove the need for screening. Screening and follow-up are tailored to your age, history and breast tissue, with ultrasound or MRI when indicated.
10. When will I see the final result?
You will usually notice a change soon after surgery, but the result develops as swelling settles and the tissues heal. Early on, the implants may look high or the breasts may feel firm. Many patients see a more settled appearance around six to eight weeks, although changes can continue for months and sometimes up to a year. These are approximate time frames, not guarantees.
11. What if I have a history of breast disease?
Tell our team about any breast condition, previous investigations or treatment before considering mia Femtech®. Your medical history needs individual review. An examination and, where appropriate, additional imaging such as ultrasound or mammography may be needed before a decision about surgery can be made.
12. Why does natural tissue coverage matter?
mia Femtech® places the implant in front of the pectoral muscle, so your own tissues need to provide sufficient coverage. You do not need a large amount of breast tissue, but its thickness and quality must be assessed.
For very thin tissues or almost complete absence of breast tissue, a submuscular or dual-plane approach may provide more coverage and help limit visible edges or rippling. Examination, imaging when appropriate and 3D simulation can help with planning; a simulation does not guarantee the final result.
13. How do surgical invasiveness and recovery compare with conventional augmentation?
Conventional surgery does not always involve cutting the pectoral muscle. mia Femtech® combines a small underarm incision, a narrow access channel and balloon expansion of the intralamellar plane. In a straightforward augmentation, it avoids extensive dissection and cuts to the gland or muscle.
Limiting dissection and preserving structures may reduce swelling, and recovery may be quicker. Many patients resume gentle everyday activities relatively soon, but 24–48 hours is not a universal recovery promise. Exercise is reintroduced gradually according to your progress.
14. Are there restrictions on physical activity after mia Femtech®?
Gentle walking and light activity are generally encouraged early in recovery, according to your individual progress and your surgical team’s instructions.
Strenuous exercise, heavy lifting and abrupt movements involving the chest are restricted during the early weeks. Dr. Aso will advise you when and how to return to each activity.
15. When can I return to strenuous exercise?
The Spanish treatment guidance gives approximately six weeks as a typical point for returning to more demanding exercise, such as running or gym workouts. This is an estimate: follow your surgeon’s clearance rather than a fixed calendar date.
Recovery differs between patients. Do not rush back to strenuous activity simply because discomfort has improved.
16. Is mia Femtech® suitable after pregnancy?
It may be appropriate when the main concern is a modest loss of volume without significant sagging. mia® adds volume, but it does not remove excess skin or, by itself, lift an areola that sits below the breast fold.
Where there is excess skin or significant drooping, a breast lift (mastopexy)may be more appropriate. Another approach that may be discussed in selected cases is Preservé breast lift. Your anatomy and the degree of sagging determine which correction is realistic. A future pregnancy can also change the result. Read our comparison of mia® and Preservé.
17. Can I have mia Femtech® after previous breast surgery?
mia Femtech® is generally not indicated after major breast procedures such as implant augmentation, reduction or a lift. Implant replacement is usually managed with conventional revision techniques.
After a more limited procedure, such as removal of a lump, suitability must be assessed individually, taking account of the operation, scars, implant plane and tissue condition.
18. Might my mia® implants need replacing in the future?
Yes. Breast implants are not lifetime devices and may require replacement or removal in the future. Ongoing checks are important. Your team will explain the follow-up plan and what to do if you notice a change or develop symptoms.
19. How does Dr. Aso view this technique?
Dr. Aso considers mia® an important development in breast augmentation because it offers selected patients a different surgical approach. This is his clinical perspective, not a guarantee of superiority. As with any technique, its benefits, limitations, indications and contraindications must be understood.
20. Could mia® suit me if I am worried about general anesthesia and a long recovery?
It may be worth discussing if you want a subtle change and are interested in a tissue-preserving approach. mia® is still surgery, and concerns about anesthesia or recovery do not establish eligibility. An individual consultation can help you understand whether mia® could suit you.
21. Will I have a 3D simulation?
The clinic uses 3D simulation to help discuss breast shape, implant options and the type of change you would like. Ask what can be assessed during an initial video consultation and what requires an in-person visit. A simulation is a planning tool, not a prediction or guarantee of the surgical result.
22. Can I sleep on my side or stomach after mia Femtech®?
During the early weeks, you will usually be advised to avoid sleeping on your chest or side to limit pressure while healing. Your surgeon will explain the recommended position, often on your back with support from pillows, and when you can return to your usual sleeping position.
23. What if I have very little breast tissue?
Some natural tissue coverage is needed for an implant placed in front of the muscle. With very thin tissue or almost complete absence of breast tissue, another approach may provide better coverage and help limit visible edges or rippling. The best option can only be determined after a personalized assessment.
