A straightforward answer

What is a BBL or buttock augmentation with your own fat?

Also known as gluteal fat grafting or a Brazilian Butt Lift, this operation combines liposuction with fat transfer. We first reshape the abdomen, flanks, back, or thighs, then prepare the harvested fat and use it to improve projection, shape, or the transition between the waist, hips, and buttocks.

It is not simply about adding volume. The outcome depends both on where fat is removed and where it is placed. Carefully planned liposculpture can make the buttocks look more balanced without aiming for an extreme increase in size.

Planning the whole silhouette

The goal is a more balanced silhouette, not the buttocks in isolation

The same amount of fat can produce very different results depending on the plan. We therefore assess the lower-back curve, flanks, hip dips, the base of the buttocks, and their relationship with the thighs.

  • Projection: adding volume where the anatomy can safely accommodate it.
  • Shape: rounding or balancing flatter areas and asymmetries.
  • Transitions: softening the junction between the waist, hips, and buttocks.
  • Donor areas: using liposuction to improve the contours surrounding the buttocks.

Realistic suitability

The key question: is there enough fat for a worthwhile change?

Suitability depends on your health, weight stability, expectations, and how much fat can be harvested without disproportionate liposuction.

It may be suitable if…

  • You want to improve projection, shape, asymmetry, or body proportions.
  • You have donor fat in the abdomen, flanks, back, or thighs.
  • Your weight is reasonably stable and you are not planning major weight loss.
  • You can avoid smoking, vaping, and nicotine products for at least one month before and one month after surgery.
  • You can follow the compression-garment instructions, pressure restrictions, follow-up appointments, and recovery schedule.
  • You accept that some fat is reabsorbed and another session may be needed.

We do not force an unsuitable procedure

We do not use this operation to treat significant obesity. Nor do we harvest fat aggressively when a patient has insufficient donor tissue.

For slim patients seeking an increase that the available fat cannot provide, we consider buttock implantsas an alternative. The options are explained during the consultation; no single technique is best for every body.

From liposculpture to fat grafting

How we perform buttock fat transfer

The operation takes approximately one to four hours. The difference mainly depends on how many areas require liposuction and the complexity of the body-contouring plan.

  1. 01

    Planning while standing

We mark the donor and recipient areas while assessing the whole body, not just the buttocks.

  2. 02

    Careful fat harvesting

We harvest fat with cannulas or microcannulas, using MicroAire or a conventional technique according to the extent of liposuction.

  3. 03

    Processing by decantation

We separate the usable fat before transferring it. Centrifugation is not a universal step in our protocol.

  4. 04

    Ultrasound-guided subcutaneous injection

Fat is placed along multiple tracks within the subcutaneous tissue, never inside the muscle, while the cannula is visualized using real-time ultrasound.

BBL safety

Subcutaneous, never intramuscular, with real-time ultrasound

Fat embolism is an uncommon but potentially fatal complication. Safety is therefore not a slogan: it means specific technical decisions.

LayerSubcutaneous tissue only

We do not inject fat into the gluteal muscle.

ImagingA visible cannula

Ultrasound lets us check the cannula’s position in real time.

LimitsNo predetermined volume target

Tissue capacity and safety take priority over a preset volume.

These measures reduce risk, but no operation can promise zero risk.

Volume and evidence

How many milliliters can be transferred to each buttock?

There is no volume target suitable for every patient. The amount depends on available fat, the elasticity and capacity of the subcutaneous tissue, the planned silhouette, and what can be transferred safely under ultrasound guidance.

For context, a European cohort using ultrasound-guided subcutaneous injection reported volumes of 500–800 mL per buttock, while a systematic review of ultrasound-guided surgery found a reported mean of approximately 704 mL. These are descriptive figures from groups of patients, not a recommendation or an individual promise.

An evolving result, not an immediate photograph

When buttock fat-transfer results become apparent

Initially, the liposuction areas are swollen and the buttocks have more volume than they will ultimately retain. Some fat is reabsorbed, while some becomes integrated as living tissue.

  1. The first weeksSwelling and adjustment

The contour changes quickly. This is not the time to judge the final volume.

  2. From the third monthAn initial, more stable assessment

If the volume is insufficient, another session can be considered from this point onward.

  3. Around 6 monthsA result that can be assessed

This is the usual time frame for evaluating fat integration and the new body proportions.

  4. Up to a yearFull maturation

The donor and recipient areas may still undergo slower changes.

Later weight changes can alter both the grafted fat and the surrounding contours. Fat transfer does not stop aging or guarantee that the volume will remain identical throughout life.

Realistic recovery

Recovery depends on both the liposuction and the buttocks

Treating more donor areas generally means more swelling and potentially more time before returning to normal activity. These guidelines are adjusted at each follow-up.

Work7–14 days

depending on the extent of liposuction and the type of work

The first days

Walking and protecting the graft

Early mobilization, wound care, hydration, and pain management. Avoid sustained direct pressure on the grafted area.

For one month

Compression over the donor areas

A compression garment is generally worn for around one month, with adjustments according to swelling, skin condition, and the liposuction areas.

4–6 weeks

Light exercise

Resume gradually when wound healing, swelling, and comfort allow.

≈3 months

Strenuous exercise

Demanding training is usually postponed until this point and always depends on your progress.

Pressure, sleeping, and driving

When can I sit after buttock fat transfer?

As a general guide, avoid direct pressure and minimize prolonged sitting during the first two weeks. When sitting is essential, place support under the thighs to take pressure off the buttocks, not directly under the grafted area.

Preferably sleep on your stomach or side. Resume driving only when you can sit with the recommended protection, get in and out of the car comfortably, react without pain, and have stopped medication that impairs your reactions. Your individual assessment takes priority over any generic timetable.

Caring for two different areas

Drainage for donor areas, protection for the buttocks

Liposuction areas

Lymphatic drainage may begin between days 5 and 7

Treatment focuses on the abdomen, flanks, back, or thighs to help manage swelling. Its frequency and duration depend on the extent of liposculpture and each patient’s response.

Some small liposuction incisions may initially be left open to allow fluid to escape. You will receive wound-care instructions; this does not itself mean that a drainage tube has been inserted.

Recipient area

The grafted buttocks are not massaged initially

During the early phase, we avoid manipulating the transferred fat. Later, the area is assessed individually to decide whether any treatment is appropriate.

No smoking, vaping, or nicotine

Our protocol requires avoiding smoking, vaping, and all nicotine products for at least one month before and one month after surgery. Nicotine and smoke impair tissue oxygenation and wound healing.

Warning signs

When to seek urgent medical care

You will receive postoperative instructions and a contact telephone number. Some symptoms should not be monitored at home or left until your next follow-up.

  • Sudden breathing difficulty or shortness of breath.
  • Chest pain, fainting, confusion, or severe palpitations.
  • Bluish discoloration, sudden weakness, or new neurological symptoms.
  • Significant pain or swelling in a leg, particularly if it affects only one side.

If symptoms suggest pulmonary thromboembolism or fat embolism: call 112 and explain that you recently had surgery. Do not drive yourself or wait for the symptoms to disappear.

Two different indications

Your own fat or buttock implants

Dr. Aso also performs augmentation with implants in selected cases. This alternative is considered particularly when there is insufficient donor fat.

Aspect Your own fat Buttock implants
Suitable candidate Has donor fat and wants to reshape several areas. Is slim or lacks enough fat for the desired change.
Main advantage Uses your own tissue and also improves the liposuction areas. Provides a defined volume without relying on donor areas.
Main limitation Some fat is reabsorbed and another session may be needed. Introduces an implant and involves its own recovery requirements.
Decision Anatomy, goals, tissues, risks, and preferences are assessed during the consultation.

Experience and setting

Dozens of procedures, performed in a full-service hospital

In our practice, we have performed dozens of these procedures. Experience does not replace patient selection or guarantee a result, but it helps anticipate how liposuction, tissues, volume, and recovery interact.

We operate under general anesthesia at Hospital Universitario HLA Moncloa. Patients usually stay in the hospital for one night of postoperative monitoring before continuing their recovery at home.

    Frequently asked questions

    Questions about buttock fat transfer

Are buttock fat transfer and a BBL the same thing?

Yes. Gluteal fat grafting, buttock fat transfer, and Brazilian Butt Lift (BBL) commonly describe augmentation or reshaping of the buttocks with your own fat. The technique includes liposuction of donor areas, fat preparation, and exclusively subcutaneous injection under ultrasound guidance.

Does all the transferred fat remain?

No. Some is reabsorbed during the first months, while some becomes integrated as living tissue. We do not promise a universal retention percentage: it depends on the tissues, volume, technique, recovery, and individual factors.

How much fat do I need to be a candidate?

No single weight or skinfold measurement answers this question. Several donor areas must be assessed to establish whether enough fat can be harvested without disproportionate liposuction. If not, buttock implants may be considered.

Is fat injected into the muscle?

No. Our protocol is exclusively subcutaneous, never intramuscular, and uses real-time ultrasound to visualize the cannula’s position during transfer.

How long does the operation take, and what anesthesia is used?

It takes approximately one to four hours, depending on the extent of liposuction. It is performed under general anesthesia and usually requires a one-night stay at Hospital Universitario HLA Moncloa.

When can I sit and sleep normally?

During the first two weeks, direct pressure and prolonged sitting are kept to a minimum. When sitting is essential, support under the thighs takes pressure off the buttocks. Preferably sleep on your stomach or side; progression is adjusted at your follow-ups.

When can I return to work and exercise?

Return to work is usually between 7 and 14 days, depending on the extent of liposuction and your job. Light exercise usually resumes after one to one and a half months; strenuous exercise, at approximately three months if recovery allows.

Will I need massage or lymphatic drainage?

Drainage of donor areas usually begins between days 5 and 7. Some small liposuction incisions may initially be left open to allow fluid to escape and need the prescribed wound care. The grafted buttocks are not treated at first; later, the area is assessed individually.

Can I have a second session?

Yes. If volume is insufficient and donor fat remains available, another session may be considered from the third month onward. The decision depends on your progress and is not scheduled automatically.

When is the result considered final?

It is usually assessed at around six months. Both the liposuction and recipient areas may continue to change more slowly for up to a year.

What does buttock fat transfer cost in Madrid?

The usual range is €7,000 to €9,500, depending on the amount of fat to be harvested and the extent of liposuction. Your assessment allows us to prepare an individual quote.
