What it involves

An areola in proportion with the breast

The areola is the circle of pigmented skin around the nipple. Its size varies naturally and can change with development, pregnancy, breastfeeding, weight changes and aging.

There is no ideal size for everyone. If you feel that your areolas are too large or uneven in relation to your breasts, areola reduction may be an option to discuss.

The procedure reduces the area of pigmented skin around the nipple. The goal is not a universal measurement, but a diameter and shape in keeping with your breast, while maintaining a natural-looking nipple–areola complex.

Your first consultation defines the plan. We assess skin elasticity, symmetry, nipple position and whether reduction should be performed alone or with another breast procedure.

Anatomical context

What is a “normal” areola size?

There is wide anatomical variation. Measurements help describe an areola, but do not by themselves establish a need for surgery.

    Small

    A small diameter can be entirely normal.

  2. 2–4 cm
    Medium

    A common range, not a required target.

  3. 4–6 cm
    Large

    This can still be normal anatomical variation.

  4. > 6 cm
    Very large

    Assessment considers the whole breast and the patient’s preferences.

Areola reduction usually reflects an aesthetic preference, rather than a medical need. The decision should follow a discussion of the scar, limitations and expected changes over time.

Surgical technique

What does areola reduction involve?

This is a minor procedure, usually performed under local anesthesia, with or without sedation, as an outpatient operation and often in less than an hour.

Two concentric circles are marked: the current diameter and the planned new border. The pigmented skin between them is removed, while the nipple and central areola remain connected to the deeper tissues.

The skin is then carefully brought together with fine sutures. A supporting suture may be used to distribute tension and limit widening. The incision lies at the edge of the new areola.

Areola reduction: before and after

Areolas may be large because of genetics or enlarge after pregnancy, weight changes and aging.

Surgery can reduce diameter and improve symmetry. The initial change is visible early, while swelling subsides gradually and the scar continues to develop over months.

Results may be long-lasting, but surgery does not stop natural changes. A future pregnancy or substantial weight change can alter the breast and areola again.

Recovery after areola reduction

Recovery is often relatively brief when the procedure is performed alone. When combined with other breast surgery, care and timing follow the needs of the full operation.

Does areola reduction leave a scar?

Yes. The scar sits around the edge of the new areola, where pigmented skin meets the surrounding breast skin. It often becomes less noticeable over time, but its quality and progression depend on tension, skin characteristics and individual healing.

Is this operation only for women?

No. Areola reduction can be performed in women and men when there is an appropriate aesthetic or reconstructive indication.

Can I breastfeed after areola reduction?

The technique aims to preserve the nipple–areola complex and its deep connections, so many patients can breastfeed afterward. However, no breast operation can guarantee that breastfeeding or sensation will be unaffected. Discuss future pregnancy and breastfeeding plans during your consultation.
