Volume where it brings balance

What is hyaluronic acid cheek filler?

This injectable treatment can restore or add volume around the cheekbones and improve the midface contour without a surgical implant.

Bone support and facial fat compartments change over time. Some people notice hollowing in the cheeks, a more pronounced transition beneath the eyes, or less definition. Others have less prominent cheekbones from a young age and would like proportionate enhancement.

At our Madrid clinic, the product we most often use is Belotero Volume, a hyaluronic acid filler from Merz. We assess how much volume is missing, where it would be helpful, and how it relates to the eyes, temples, and the rest of the face. We agree on the goal before treatment.

What cheek filler can improve

The same volume can have different effects depending on its placement. Planning begins with your concerns and facial proportions.

A natural-looking result depends on respecting anatomy, expression, and existing volume, in both women and men.

  1. 01

    Restore lost volume

Selectively replace reduced support in the cheekbones or cheeks, without filling the entire midface uniformly.

  2. 02

    Enhance flatter cheekbones

Improve projection and definition when your starting anatomy and goals make adding volume appropriate.

  3. 03

    Balance an asymmetry

Plan each side individually. Faces are not perfectly symmetrical, and correction does not always require the same amount on each side.

  4. 04

Soften the transition to the cheek

In selected cases, cheekbone support may improve the transition beneath the eyelid or the appearance of the nasolabial fold. Under-eye hollowing and the fold are also assessed separately.

The product and its role

Belotero Volume: hyaluronic acid for support

Hyaluronic acid occurs naturally in tissues, where it helps retain water. In fillers, it is formulated as a gel for placement in specific areas to add volume; its behavior depends on the formulation and injection site. A volumizing treatment is not the same as a treatment intended to improve superficial skin hydration.

Belotero Volume is the product we most often use in this area. We select it for the support and tissue integration we aim to achieve, after assessing the skin, bone structure, and soft tissues. Not every product in the Belotero range has the same role.

A comparative trial of Belotero Volume for cheek volume loss observed persistence of the effect for up to 18 months. These findings relate to a specific study population and protocol, not an individual guarantee. [1]

Support

The choice of product and injection plane reflects the intended change in each part of the cheekbone.

Precision

Assessment distinguishes projection, volume restoration, and smoother transitions; adding more filler is not the answer to every concern.

Resorption

The result is temporary. Maintenance is considered after assessing changes, rather than at a fixed interval for everyone.

Our approach to volume

How many cc are needed for cheek enhancement?

In our practice, the usual amount is approximately 1 cc per side. The range may extend from 0.5 cc per side for a small correction to 2 cc per side for severe volume loss in carefully selected cases.

One cc is equivalent to one milliliter. These figures describe the range we work with; the appropriate amount is determined after facial examination and may differ between the right and left sides.

Our approach in the clinic

How cheek filler is performed

This is an outpatient treatment. Injection usually takes 15 to 20 minutes; assessment, planning, and the time needed for the anesthetic cream to work are additional.

  1. 01

    Examination and goals

We review your medical history, medications, and previous fillers. We examine your face from the front, side, and three-quarter views, at rest and with expression, and agree on the areas to treat.

  2. 02

    Preparation and anesthetic cream

We prepare the skin and apply anesthetic cream to improve comfort. You may feel a needle prick, pressure, or discomfort during treatment; our aim is to make the experience tolerable.

  3. 03

Needle and cannula according to the goal

We use both and combine deep planes near the bone or within fat compartments, depending on anatomy and the support needed. The technique is tailored to each area and performed under aseptic conditions.

  4. 04

    Assessment and follow-up

We check the initial contour and explain aftercare. We schedule a review at two weeks to assess settling and whether any adjustment is appropriate.

Visible changes, gradual settling

Hyaluronic acid cheek filler: before and after

The added volume is visible at the appointment. The contour is easier to assess once initial swelling has subsided.

During the first few days, your cheekbones may look fuller than expected, or one side may be more swollen. Changes over time and the follow-up examination guide any adjustment, rather than a photograph taken immediately after treatment.

We usually assess the result between 7 and 14 days, with a review at two weeks. If swelling or discomfort persists, we adjust follow-up before considering more product.

A temporary result

How long does hyaluronic acid cheek filler last?

As a guide in our practice, between 12 and 18 months.

Duration depends on the product, amount, anatomy, and individual response. The effect changes gradually; it does not disappear abruptly at a set time.

Maintenance is considered when examination indicates it is appropriate. Before adding filler, we assess how much support remains and how the face has changed. A two-year volumetric follow-up study shows that volume may persist beyond commonly quoted durations; previous filler should therefore not be assumed to have completely resorbed. [2]

Recovery with support

Aftercare following cheek filler

You can usually resume gentle everyday activities. Allow time before an important event, as bruising or swelling may occur.

These general guidelines draw on information from Belotero and the American Society of Plastic Surgeons. They are tailored to the product used and your recovery. [3] [4]

  • First 12 hours: avoid makeup over the treated sites; keep your hands and the area clean.
  • First 24–48 hours: avoid strenuous exercise. If swelling persists, ask before resuming it.
  • Do not massage or press on your cheekbones unless instructed by your doctor. Also avoid devices or facial treatments that compress the newly treated area.
  • For two weeks: avoid saunas, steam baths, and prolonged sun or UV exposure, in line with Belotero recommendations.
  • If you have an upcoming dental procedure, let us know beforehand so we can coordinate timing. Do not delay urgent dental care.
  • Regular medication: tell us about anticoagulants, antiplatelet medications, and supplements. Do not stop prescribed medication on your own.

When treatment is appropriate

Who may be a suitable candidate?

It may be an option for adults with volume loss, less prominent cheekbones, or an asymmetry that filler may balance, provided expectations are realistic.

Already full cheeks, prominent eye bags, or substantial skin laxity require assessment of other causes and alternatives. Adding more filler does not replace that diagnosis.

If you already have fillers, tell us which product was used, when, and in which areas. This is particularly important if the material is permanent or unknown.

Choosing the right option

Hyaluronic acid, your own fat, or cheek implants

These treatments share an anatomical area, but they are different procedures. The choice depends on your anatomy, desired change, and whether you prefer a temporary injectable treatment or a surgical option.

Differences between hyaluronic acid filler, fat grafting, and cheek implants
Option What it involves What to consider
Hyaluronic acid An injectable filler to add volume and support in the clinic. A temporary result with individualized adjustment. It can be dissolved with hyaluronidase when medically indicated.
Your own fat Transfer of fat obtained by liposuction from a donor area. Requires surgery, available donor fat, and a different recovery. Some of the fat may resorb.
Cheek implants Surgery to change projection using an implant. Considered for specific structural goals; involves surgery and implant-related risks.

Individualized facial balance

Can it be combined with other treatments?

Yes. In our practice, cheek assessment can be combined with assessment of the under-eye area, nasolabial folds, jawline, lips, or temples when appropriate. Each area requires its own product, technique, and evaluation.

Treating several areas is not a requirement for a balanced result. We can prioritize what is most helpful and organize treatment in stages.

Medical judgment and follow-up

Safety and reversibility

The most common effects are temporary local reactions: swelling, tenderness, or bruising. Irregularities, nodules, infection, or inflammatory reactions can also occur, sometimes with delayed onset.

Uncommonly, vascular occlusioncan occur, compromising the skin and, exceptionally, vision. Recognizing the signs helps enable prompt action. [5]

The clinic has hyaluronidase available to dissolve hyaluronic acid when medically indicated, in accordance with AEMPS recommendations. More than one treatment may be needed, and it carries its own risks; having it available does not eliminate every complication. [6]

    Answers to help you decide

Frequently asked questions about cheek filler

Which hyaluronic acid does Dr. Aso use for cheek enhancement?

The product we most often use is Belotero Volume from Merz. The choice is tailored to anatomy, the support required, and previous treatments.

How many cc are needed per side?

In our practice, the usual amount is approximately 1 cc per side. We may use from 0.5 cc per side for small corrections to 2 cc per side in selected severe cases. The amount may differ between the right and left sides.

    Does cheek filler hurt?

We apply anesthetic cream to improve comfort and use a needle, cannula, or both as appropriate. You may feel a needle prick, pressure, or some discomfort, followed by tenderness in the area.

How long does the appointment take?

Injection usually takes 15 to 20 minutes. Assessment, preparation, and the time needed for the anesthetic cream to work are additional.

When can the settled result be assessed?

The volume change is visible at the appointment. We usually assess the contour between 7 and 14 days and review at two weeks. If swelling persists, we adjust follow-up before deciding on any further treatment.

How long does hyaluronic acid cheek filler last?

As a guide in our practice, between 12 and 18 months. Duration varies with the product, amount, anatomy, and individual response. Maintenance is considered after examining how much volume remains.

    Can the result look natural?

The aim is to improve support or projection while preserving your expression. We assess the whole face and tailor the amount and placement. Not everyone needs the same volume or treatment of every area.

Does cheek filler eliminate under-eye hollowing or nasolabial folds?

In selected cases, improving cheekbone support may soften these transitions. It does not independently correct all under-eye hollowing, bags, wrinkles, or skin laxity. Each concern is examined and may need specific treatment.

    Who can have this treatment?

Adults with volume loss, less prominent cheekbones, or asymmetries that may improve with filler, and realistic expectations. Before treatment, we review medical history, medications, allergies, and previous fillers.

Can hyaluronic acid be removed if needed?

It can be dissolved with hyaluronidase when medically indicated. The clinic has it available. Several treatments may be needed, and the procedure carries its own risks; it does not guarantee immediate reversal of every problem.

Can I exercise or wear makeup afterward?

As a guide, avoid makeup over the area for the first 12 hours and strenuous exercise for 24–48 hours. Do not massage or press on your cheekbones unless instructed by your doctor, and adjust your return to activities to your recovery.

How much does hyaluronic acid cheek filler cost?

The indicative price at our Madrid clinic is 500 EUR. At assessment, we agree on the product, amount, areas, and scope of the quote before you decide.
